Will cleaning lower photo quality?

Privacy Clean does not recompress JPEG image data. Full Flatten can change compression and file size.

Why can orientation be preserved?

Some photos store sideways pixels and rely on an orientation instruction. A minimal safe orientation tag prevents unexpected rotation.
